Apprenticeship Request to Register Form

Complete the following form and click Submit at the bottom of the page. Whitby Enrolment Services will email you once you have been registered, noting registration is first come, first served and only by this form due to our remote services at this time.  If you have any questions please contact us through the LiveChat option or email wes@durhamcollege.ca. 

Registrations for the August and September apprenticeship intakes has opened. Please fill out the apprenticeship registration form below and you will be placed in order of receipt.  You will receive a response within 5 to 7 business days

    • Registration is first come, first served and only by email at this time
    • WES will send you a confirmation email once registered
    • This email will indicate the exact charge to your account and may include tuition as noted in your Ministry Offer email, HST and a $14 transcript fee and grad fee for Advanced or Level 3 .
    • Apprenticeship fees are assessed to your student account immediately upon registration
    • Payment is required within five (5) days of the date of our email to you, or you may be deregistered
    Payment Options
    • Credit Card (Visa, MC, AMEX) on MyCampus.
    • Bank Transfer (takes 3-5 business days) - use your Durham College student number for the account number
    • Personal cheques or cash payments are not accepted
    SIN Collection
    • CRA (Canada Revenue Agency) requires your SIN (Social Insurance Number) for your T2202 tax form
    • Add your SIN on MyCampus if you have not already done so, or submit it as soon as possible
    Withdrawal
    • You are academically and financially responsible for your program unless you officially withdraw before the start of the first class. Failure to attend does not constitute withdrawal. To withdraw, a complete Program Withdrawal Form must be emailed to wes@durhamcollege.ca  by the start of the first class and is subject to a $100 admin fee.
    • After the start of the first class no refunds will be considered.
    • Students who stop attending and do not follow proper withdrawal procedures will be recorded as having failed the program.
