Confidentiality

When booking an appointment, students are asked the nature of the visit, so that an appropriate amount of time can be booked for that appointment. For example, mental health and periodic health visits (PHV) require longer appointment times. Only general information is required to assign the appropriate amount of time for your appointment. You do not need to disclose the entire nature of your visit to the reception staff.

All services provided at Health Service are confidential. If you request a student medical certificate/sick note for academic consideration, only the information agreed upon by you and your doctor will be included on the form. No one from an academic or administrative department has access to your health record unless you’ve granted permission. Parents, guardians and other family members cannot have access to any information about you, including whether or not you have ever visited the Campus Health Centre, without your expressed or written consent.

Personal Health Information Protection Act, 2004 (PHIPA) is Ontario’s new health-specific privacy legislation. PHIPA governs the manner in which personal health information may be collected, used and disclosed within the health care system.
