Social Services Worker Field Placement FAQs What documents are required for my field placement? The following are the documents required for each of the SSW field placements. Upload to DC Connect: SSW Field Placement application form Remote Work Health and Safety Pre-Placement Checklist (for remote and hybrid placements only) Upload to Verified by Synergy Gateway: Entry Immunization Form Criminal Reference Check with Vulnerable Sector Screening (Valid within Six Months from the start of placement term – see PPL website for specific date ranges) Standard First Aid & Level C CPR (Note: Online and Hybrid (online & in-class) courses will only be accepted if the course is WSIB approved). Student Declaration of Understanding Informed Consent for the Release of Personal Information COVID-19 Informed Consent and Assumption of Risk form Online Training Modules: (Accessed via PPL’s DC Connect Application Modules) Workplace Hazardous Materials Information (WHMIS) Module Completion Certificate Accessibility for Ontarians with Disabilities (AODA) Module Completion Certificate Workplace Violence and Harassment Prevention Module Completion Certificate Worker Health and Safety Awareness in 4 Steps Module Completion Certificate Diversity Module Completion Certificate Where do I find the Health and Safety & Diversity certificates I’m required to complete? These certificates are included as part of the Pre-placement Paperwork Modules, free of charge. Once you have requested activation and have been granted access to the Pre-Placement Paperwork module, you will have access to complete the Health and Safety and Diversity certificates. Only these Durham College certificates will be accepted as part of the placement clearance requirement. How many hours must I complete for placement? Students are required to complete the minimum number of hours, as indicated below, and must be completed and must be completed by the course end date as advertised. Students will be required to complete seminar hours online, journals and actively participate in online discussions in order to be successful in this placement. Each SSW Field Placement is broken down as follows: Hours completed at the host setting: 210 hrs Hours allotted for online seminar: 14 hrs Hours allotted for weekly journals: 14 hrs Total = 238 hrs Students must complete ALL required hours & final evaluation on or before the published course end date. Failure to do so will result in the student receiving an unsatisfactory grade for the field placement course. Am I required to find my own placement? Yes. Students in the Social Service Worker program are required to research and secure each of their placements. This may require an in-person interview with the agency supervisor prior to being accepted at the agency. What credentials does my agency supervisor need to have? Direct supervision, while on placement, should be completed by a field placement supervisor who is appropriately credentialed. Your supervisor must have: Social Work, Social Service Work, Psychology, Sociology, Addictions and/or Mental Health credentials and is an experienced employee of the placement agency familiar with the social services field. Can I do my placement in a school setting? SSWs are no longer employed in school settings, therefore, students should refrain from attempting to secure school placements. Many school boards will not respond to students requesting SSW placements for this reason. Instead, students should research community social service agencies to find their placements. Can I do my placement in another province? Field Placements in this program can be completed in another province. May I do all of my placements at the same setting? Students are required to secure a placement agency for Levels 2 and 3 that is different than the agency secured for Level 1. In addition to meeting the learning outcomes, this also provides a greater variety in experience and greater flexibility in the job market. Please note: Each placement level must be done in separate semesters. Paperwork requirements and deadlines will apply to each placement/term separately. Am I able to challenge any/all of my field placements? All Level 1, 2 and 3 placements are eligible for PLAR.
