CNO Exam Candidate Information Form

Please complete the survey with your personal information. This information will be sent to the CNO to prepare for your registration. Once you have completed the Practical Nursing program, this same information will be sent to the CNO so you can register for your exam.

You must ensure the email you input into this information form is the same email you will continue to use to complete your CNO registration.

You must ensure that the name you put int this email matches the name on your legal documents/identification.

NOTE: The timeline for the CNO verification process may take up to three weeks after your final grade has been released. You will be notified by the faulty office when you name has been submitted to the CNO.
