  • Creed: Religious, spiritual or sincerely held belief system
  • Family status: Responsibilities involving family caregiving or dependents

We encourage you to complete these steps as soon as you have accepted your Offer of Admission or are already enrolled in courses. You can connect with the ASC anytime throughout your academic journey.

Step 4: Submit Your Forms
Send your completed forms and documents to the ASC. You can submit them:

  • By email
  • By fax
  • In person

If you are not sure what to send or how to register, we can help you. Visit the contact page to get support.

After You Apply

Once your forms and documents are submitted, you will receive a confirmation email. Shortly after, ASC staff will contact you to book your intake appointment.

During your intake appointment, you will meet with a staff member to talk about the support you need. Together, you will review possible accommodations and create your Access Plan.

In college, your instructors/professors will not know you need accommodations unless they are notified. The ASC will share your Access Plan with your instructors/professors each semester by email, so they understand your learning needs.

If you have questions at any time, the ASC team is here to support you.
