Campus Walk card photo submission

As a student who has been hired for the Campus Walk program, we require that you carry Office of Campus Safety (OCS) issued identification for service under this program. While on duty on campus for your scheduled shifts, you will use this card to identify yourself to the campus community.

You must complete the form below, and you must submit a photo to be used on your OCS issued Campus Walk card. To avoid having your photo rejected, please make sure to follow our photo upload requirements. Otherwise, you’ll be required to submit a new one.

This is your opportunity to take or choose the photo which will appear on your Campus Walk Card. There are specific photo requirements, please make sure you follow the instructions listed below. If your photo does not follow these guidelines, it will be rejected, and you will be required to resubmit.

Photo upload requirements:
- Background must be white or a light solid colour.
- Photo must be in colour.
- You must be the only person in the picture.
- Photo must show a full, front view of the face and top of shoulders, squared to the camera.
- No hats, head coverings (except for religious purposes) or sunglasses.
- No inappropriate clothing or gestures (i.e. If you would not share this photo with a parent/guardian or future employer, you should not submit it to us).
- No photos from graduation.
- Feel free to smile.

This card is the property of Durham College, and must be surrendered on request by Campus Security or a campus official.
The person identified by this card is fully responsible for transactions made as a result of presenting the card, for use of campus property & resources, or any associated privileges.
Use of this card is governed by Durham College policies.
Lost, found or damaged cards must be reported immediately to the Campus Identification Services Office or Campus Security.
Durham College assumes no liability arising out of, or in respect of third party services facilitated through use of this card.
Replacement fees for DC issued cards are the responsibility of the student to whom the card has been issued.
Misuse of the DC Card can result in loss/suspension of privileges, or sanctions as described in DC policies.
